To get started on your personalized quote, follow these steps:

1. Download the Group Census Form by clicking the "Census Form" button below

2. Complete the form and save it as a password protected excel file (see form on how to save as pw protected file)

3. Return the form via email to Sam@STownsendInsurance.com - Do not send the password in the email with the completed form

4. Send the password for protected excel file in a separate email (do not title email "Password")
